THE ID NUMBER IS THE EMPLOYEE’S SOCIAL SECURITY NUMBER
THE GROUP NUMBER IS 2727
THE BIN NUMBER IS 600471
This information is also located on the back of the patient’s ID card.
If you are having problems with your prescription or if you have questions about why your prescription claim was denied please call 801-972-1177 or 800-657-5377 and ask for pharmacy assistance.

If the prescription is being filled by Mail Order:
CLAIMS WILL BE PROCESSED THROUGH
UNION CENTER PHARMACY
(If you have prescriptions that have been prescribed for long term use
that are filled on a regular basis, this option may help you save money!)
Your doctor can call, fax or E-scribe (electronically prescribe) new
prescriptions to Union Center Pharmacy to be filled.
Re-fills can then be ordered by mailing in a mail order envelope,
or for fastest service you may request a refill online.
